ITI Biodata Format — With a Full Filled Sample

Full name exactly as written on your Class X certificate, father's or husband's name in full, date of birth taken from that same certificate, gender, marital status, nationality, category where the form asks for it, languages known, and both a permanent and a present address. Do not fill permanent and present with the same text out of habit — permanent is the address used for domicile and postal verification, present is where you can actually be reached now. If they genuinely are the same, write "Same as above".

This is the part a resume has no equivalent for and the part ITI candidates most often under-fill. Give the trade name exactly as printed on the certificate, the duration, whether it is NCVT or SCVT, the ITI name and district, the year of passing, and your marks or grade. Add the certificate or roll number if you have it — recruitment offices verify against it, and a bio-data carrying the number saves a round of correspondence. NCVT is the national-level certificate and is accepted across states and by most PSU and railway apprenticeship schemes; SCVT is issued by a state council. Leaving that one word out is the most common reason an ITI application stalls.

Almost every ITI passout has an apprenticeship and almost none of them write it properly. Give the establishment name, the department or line you were placed in, the period, and what you actually did there — the machines or systems, what you assisted on at the start and what you handled unsupervised by the end. If you are registered under an apprenticeship scheme, include the registration number. An apprenticeship written as one line reads as time served; written as work, it reads as a year of experience, which is what it was.

Below the trade section, a plain table for Class X, Class XII or diploma if applicable, with board, year of passing and percentage. Keep it in a bordered table rather than as prose — offices copy these figures onto their own sheets and a table is what they are expecting to read from. Do not omit Class X: for most ITI trades it is the eligibility qualification and a missing row is treated as a gap rather than an oversight.

Category and sub-category where asked, domicile state, employment exchange registration number if you hold one, driving licence and class, any safety or skill certification, and physical details where the post requires them. For PSU and railway applications these are not optional extras — they are eligibility fields, and a blank one gets the form set aside rather than queried.

A bio-data ends with a signed declaration. For government and PSU applications use the fuller wording, which adds the consequence clause: "I hereby declare that all the statements made in this bio-data are true, complete and correct to the best of my knowledge and belief. I understand that in the event of any information being found false or incorrect, my candidature is liable to be cancelled." Place and date on the left, signature on the right, in ink, on every printed copy.
