On company letterhead, signed by HR or the manager.

When you get it
Companies usually issue the relieving letter on or after the last working day, once the notice period, handover and clearance are done. Full and final settlement may come later, as per company policy. If the letter is delayed, ask HR in writing for the expected date.
Relieving letter or experience letter?
The relieving letter is about leaving: resignation accepted, released from a date. The experience letter is about the work: role, duties and period. Some companies issue a single letter that covers both.
