Hospitals hire for many posts at once: staff nurse, ward boy or ward aaya, receptionist, lab assistant, pharmacy assistant, billing and housekeeping. The biodata that works for all of them puts the post, your qualification or registration, and the department you have worked in near the top.

Hospitals read biodatas by post. Put the post applied for at the top, then the department you know: OPD, ICU, casualty, OT, ward, lab or pharmacy.
For nursing and pharmacy posts, the council registration number is the first thing checked. For lab posts, write the exact course name (DMLT or B.Sc MLT). For ward and housekeeping posts, any patient care or first aid training helps.
Hospitals run day and night shifts. State the shifts you can do; it often decides who gets called first.
Qualification certificates, council registration if any, experience letters, Aadhaar (masked if you prefer), two photographs, and a medical fitness or vaccination certificate if the advertisement asks.
