Pharmacist Biodata Format for Hospital, Retail and Government Pharmacy Jobs
A pharmacy can only dispense medicines under a registered pharmacist, so the first thing an employer checks on your biodata is your qualification and your State Pharmacy Council registration number. After that it looks at where you have worked: a hospital pharmacy, a retail chemist or a manufacturing unit. This format puts those lines at the top.
When this format is the right one
Use it for a pharmacist, hospital pharmacist, retail pharmacist, pharmacy assistant, dispensing chemist or government pharmacist post that asks for a biodata, and for a chemist shop that wants your details on one page.

Write D.Pharm or B.Pharm exactly as on your certificate, then your State Pharmacy Council and registration number. If your registration is in process, write the application date; do not write a number you do not have yet.
Hospital, retail and manufacturing pharmacy are different jobs. One line naming the setting, plus the work you did there (dispensing, stock, cold chain, records), tells the employer where you fit.
Chemist shops and hospitals are inspected. A line on the Schedule H1 register, expiry checks and the billing software you used reads as someone who keeps the pharmacy out of trouble.
Pharmacy marksheets and certificate, council registration certificate, training or experience certificates, 10th and 12th marksheets, an ID proof and two photographs.
What to get right

- Name the setting: hospital, retail or manufacturing.
- Add one line on records and the software you used.

Common questions
What should a pharmacist biodata contain?
Post applied for, name, age, contact, D.Pharm or B.Pharm with institute and year, State Pharmacy Council registration number, the pharmacies you have worked in and what you did there, education, documents enclosed and a signed declaration.
Can I apply before my pharmacy council registration comes through?
Yes, but say so honestly: write "registration applied on [date]". Most employers will need the number before you can dispense on your own.
I am a D.Pharm fresher. What do I write under experience?
Your practical training: the hospital or pharmacy, the months, and the work you did there, such as dispensing under supervision and stock checks.
Biodata or resume for a pharmacist job?
Government hospitals and many chemist shops ask for a biodata; pharmacy chains and pharma companies usually want a resume, which the pharmacist resume page covers.
