Railway Apprentice Medical Certificate: Format and Fitness Standards
Every railway zone asks shortlisted act apprentices for a medical fitness certificate at document verification. The form follows Schedule II of the Apprenticeship Rules, so the standards are the same across zones even though each zone prints its own format. This page lists those standards in plain words and tells you who can sign the form.

Download the medical certificate attached to your zone's notification or call letter and print it as instructed. North Eastern Railway, for example, asks for it on one page printed on both sides, with your photo attested by the medical officer. Central Railway attaches its format as an annexure to the notice, and Western Railway's 2022 notice called it Annexure G.
Who signs it
The NER format says the certificate must be signed by a government authorised doctor (gazetted), not below the rank of Assistant Surgeon of a central or state hospital. A private clinic's certificate is not enough unless your notice says so.
Height, weight and chest
The minimum is 137 cm height and 25.4 kg weight, with chest expansion of at least 3.8 cm. A candidate below these minimums can still be engaged if a medical officer not below Assistant Surgeon (gazetted) certifies in writing that the candidate is fit for that trade.
The other standards on the form
Eyes: vision in one eye is allowed except in 17 listed trades (such as Surveyor, Electrician Aircraft and Driver cum Fitter), and colour vision is not required. Ears: hearing must be good in both ears, and no hearing aid is allowed. Blood pressure must not exceed 140 systolic or 85 diastolic, and a systolic reading below 100 is a reason for rejection.
The form also checks for contagious disease and tuberculosis, skin disease, sugar in the urine, enough natural teeth, and normal limbs, spine, chest and nervous system.
